Applications MUST BE SENT BY FIRST CLASS MAIL and MUST BE RECEIVED
by the deadline date. No applications received after the deadline will be considered.

Application Documents:

1. Application form with two self addressed envelopes (download application form)
2. Transcript(s) of previous two years
3. Two letters of recommendation (download recommendation form)
4. One photo of applicant taken within past 12 months
5. A copy of most current student's, parents' or guardian's income tax file
6. Student's essay (use application form)
